Abstract Background Cardiovascular complications of COVID-19 are important aspects of the disease’s pathogenesis and prognosis. Evidence on the prognostic role of troponin and myocardial injury in Latin American hospitalized COVID-19 patients is still scarce. Objectives To evaluate myocardial injury as independent predictor of in-hospital mortality and invasive mechanical ventilation support in hospitalized patients, from the Brazilian COVID-19 Registry. Methods This cohort study is a substudy of the Brazilian COVID-19 Registry, conducted in 31 Brazilian hospitals of 17 cities, March-September 2020. Primary outcomes included in-hospital mortality and invasive mechanical ventilation support. Models for the primary outcomes were estimated by Poisson regression with robust variance, with statistical significance of p<0.05. Results Of 2,925 patients (median age of 60 years [48-71], 57.1% men), 27.3% presented myocardial injury. The proportion of patients with comorbidities was higher among patients with cardiac injury (median 2 [1-2] vs. 1 [0-2]). Patients with myocardial injury had higher median levels of brain natriuretic peptide, lactate dehydrogenase, creatine phosphokinase, N-terminal pro-brain natriuretic peptide, and C-reactive protein than patients without myocardial injury. As independent predictors, C-reactive protein and platelet counts were related to the risk of death, and neutrophils and platelet counts were related to the risk of invasive mechanical ventilation support. Patients with high troponin levels presented a higher risk of death (RR 2.03, 95% CI 1.60-2.58) and invasive mechanical ventilation support (RR 1.87, 95% CI 1.57-2.23), when compared to those with normal troponin levels. Conclusion Cardiac injury was an independent predictor of in-hospital mortality and the need for invasive mechanical ventilation support in hospitalized COVID-19 patients.

Resumo Fundamento As complicações cardiovasculares da COVID-19 são aspectos importantes da patogênese e do prognóstico da doença. Evidências do papel prognóstico da troponina e da lesão miocárdica em pacientes hospitalizados com COVID-19 na América Latina são ainda escassos. Objetivos Avaliar a lesão miocárdica como preditor independente de mortalidade hospitalar e suporte ventilatório mecânico em pacientes hospitalizados, do registro brasileiro de COVID-19. Métodos Este estudo coorte é um subestudo do registro brasileiro de COVID-19, conduzido em 31 hospitais brasileiros de 17 cidades, de março a setembro de 2020. Os desfechos primários incluíram mortalidade hospitalar e suporte ventilatório mecânico invasivo. Os modelos para os desfechos primários foram estimados por regressão de Poisson com variância robusta, com significância estatística de p<0,05. Resultados Dos 2925 pacientes [idade mediana de 60 anos (48-71), 57,1%], 27,3% apresentaram lesão miocárdica. A proporção de pacientes com comorbidades foi maior nos pacientes com lesão miocárdica [mediana 2 (1-2) vs. 1 (0-20)]. Os pacientes com lesão miocárdica apresentaram maiores valores medianos de peptídeo natriurético cerebral, lactato desidrogenase, creatina fosfoquinase, N-terminal do pró-peptídeo natriurético tipo B e proteína C reativa em comparação a pacientes sem lesão miocárdica. Como fatores independentes, proteína C reativa e contagem de plaquetas foram relacionados com o risco de morte, e neutrófilos e contagem de plaquetas foram relacionados ao risco de suporte ventilatório mecânico invasivo. Os pacientes com níveis elevados de troponina apresentaram um maior risco de morte (RR 2,03, IC95% 1,60-2,58) e suporte ventilatório mecânico (RR 1,87;IC95% 1,57-2,23), em comparação àqueles com níveis de troponina normais. Conclusão Lesão cardíaca foi um preditor independente de mortalidade hospitalar e necessidade de suporte ventilatório mecânico em pacientes hospitalizados com COVID-19.

Abstract Healthcare workers present an increased risk of contagion for the SARS-CoV-2 virus due to their labor exposure. Here, we describe the clinical, laboratory, and immunological characteristics of healthcare workers, before vaccine application, exposed to SARS-CoV-2-infected patients. We collected sociodemographic, clinical, and laboratory information from 50 professionals who worked during the COVID-19 pandemic at the Clinical Hospital of the Northwest in Brazil. The results showed that most workers are women, over 50 years old, and worked as nursing technicians. Approximately 56% of workers were positive for a previous infection by RT-PCR and/or anti-SARS-CoV-2-immunoglobulin tests. Increased levels of hematocrit, neutrophils, NK lymphocytes, and fibrinogen, were found in positive healthcare workers, suggesting a light inflammatory status. The immunological findings showed an increase in IL-17 production and a Th2/Th17/Th22 profile followed by high serology for anti-SARS-CoV-2 IgM and IgG. Those data reveal the importance of studies with healthcare workers to investigate if the continuous exposition to the virus may result in chronic activation of the immune system and/or pulmonary inflammation in this target group.

ABSTRACT: From a black decolonial perspective (GOMES, 2020), this article aims to discuss the paths taken by the Center for Afro-Brazilian and Indigenous Studies of the Colégio Pedro II (NEABI/CPII), in Rio de Janeiro, Brazil, in the (re)production of emancipatory and anti-racist practices and knowledge. In methodological terms, the research was structured based on the narratives of four black intellectuals committed to anti-racist education, who actively participated in the creation process of the NEABI/CPII, in 2013, and who had a recognized role in the constitution of practices based on (re)education for ethnic-racial relations and the fight against racism. In summary, we observed that the black intelligentsia underlying the group establishes a position of insubordination in relation to typical coloniality practices in the (re)production of knowledge, with actions that unfold into four dimensions: political-institutional, political-epistemological, political-identity and political-pedagogical. We understand that these dimensions, which are articulated with each other, are crucial for the constitution of educational spaces that are more democratic, racially diverse and politically positioned in the fight against inequalities

RESUMEN: A partir de una perspectiva negra decolonial (GOMES, 2020), este artículo tiene como objetivo discutir los caminos del Núcleo de Estudios Afrobrasileños e Indígenas del Colégio Pedro II (NEABI/CPII), en Rio de Janeiro, Brasil, en la (re)producción de prácticas y conocimientos emancipatorios y antirracistas. En términos metodológicos, la investigación se estructuró a partir de las narrativas de cuatro intelectuales negros comprometidos con la educación antirracista, que participaron activamente del proceso de creación del NEABI/CPII, en 2013, y que tuvieron un papel reconocido en la constitución de prácticas basadas en la (re)educación para las relaciones étnico-raciales y la lucha contra el racismo. En suma, notamos que la intelectualidad negra que conforma este colectivo se encuentra en posición de insubordinación frente a prácticas de la colonialidad en la (re)producción de saberes, con acciones que se desarrollan en cuatro dimensiones: político-institucional, político-epistemológico, político-identitaria y político-pedagógica. Entendemos que estas dimensiones, que se articulan entre sí, son cruciales para la constitución de espacios educativos más democráticos, racialmente diversos y políticamente posicionados en la lucha contra las desigualdades.

Em termos metodológicos, a pesquisa foi estruturada com base nas narrativas de quatro intelectuais negros/as comprometidos/as com a educação antirracista, que participaram ativamente do processo de criação do NEABI/CPII, em 2013, e que tiveram reconhecido protagonismo na constituição de práticas calcadas na (re)educação para as relações étnico-raciais e no combate ao racismo. ABSTRACT Resilience is a concept used to reflect on how to deal with the dynamics that arise after disturbances caused by a natural hazard. It emerges as a key concept for thinking about forms of recovery and reconstruction that seek not only to return to pre-disaster normality, but to question and overcome the vulnerability. Based on the Pecheutian discourse analysis method, the concept of resilient cities is problematized, mobilized as a sign of fictitious capital in the era of financialization of housing and urban space, transforming environmental disasters and the climate crisis into yet another business model. Rescuing the meaning of socio-ecological resilience and articulating it to the theories of socio-spatial practices of resistance and common spaces, it becomes possible to think of resilience to disasters not as a neoliberal category, but as a social, political, and collective practice of adaptation and mitigation to climatic changes.

RESUMEN Resiliencia es un concepto utilizado para reflexionar sobre cómo hacer frente a las dinámicas que surgen después de las perturbaciones causadas por un peligro natural. Surge como un concepto clave para pensar formas de recuperación y reconstrucción que busquen volver a la normalidad previa al desastre, y también cuestionar y superar la vulnerabilidad. A partir del método de análisis del discurso pecheutiano, se problematiza el concepto de ciudades resilientes, movilizado como signo de capital ficticio en la era de la financiarización de la vivienda y el espacio urbano, transformando los desastres ambientales y la crisis climática en un modelo de negocio. Rescatando el significado de resiliencia socioecológica y articulándolo a las teorías de prácticas socioespaciales de resistencia y espacios comunes, se hace posible pensar la resiliencia no como una categoría neoliberal, sino como una práctica social, política y colectiva de adaptación y mitigación a lo cambio climático.

RESUMO Resiliência é um conceito que busca refletir sobre como lidar com as dinâmicas que surgem após perturbações causadas por um evento natural perigoso (como deslizamentos). Ela surge como um conceito-chave para se pensar formas de recuperação e reconstrução que buscam não somente retornar à normalidade pré-desastre, mas questionar e superar a vulnerabilidade das populações expostas. A partir do método de análise de discurso pecheutiana, problematiza-se o conceito de cidades resilientes, mobilizado como um signo de capital fictício na era da financeirização da moradia e do espaço urbano, transformando os desastres ambientais e a crise climática em mais um modelo de negócios. Resgatando o significado da resiliência socioecológica, e articulando-o às práticas socioespaciais da resistência e aos espaços comuns, torna-se possível pensar a resiliência a desastres não como uma categoria neoliberal, mas como uma prática social, política e coletiva de adaptação e mitigação às mudanças climáticas globais.

Sodium chloride is used in the cheese salting process as it promotes sensory changes and food preservation. However, in excess can cause hypertension problems, and for this reason, it has been partially replaced by potassium chloride. In the present work, Prato cheese was subjected to joint diffusion of NaCl and KCl by immersion in static and stirred brine. The salt concentration values on the cheese surface were determined as a function of time. They were tabulated and presented to the self-organizing map (SOM)-type neural networks and the multilayer perceptron (MLP) for analysis and modeling of the film formed on the surface of the cheese during static and dynamic salting. The SOM network showed that the behavior of the diffusive process, on the surface of the cheese, depends on the analyzed position and that the points in which occupy similar positions, concerning the distance from the edge, have similar behavior. Regression models for NaCl and KCl were significant at the 5% level and can be used for predictive purposes.

The NaCl and KCl diffusion in the film formed on the cheese surface during salting was simulated by the finite element method. The time and salts concentration values on the cheese surface were determined, tabulated, and presented to the multilayer perceptron neural network (MLP) for the regression modeling. The samples were divided into 70, 15 and 15% for training, testing, and validation, respectively. The networks with the best performance showed 5 to 12 hidden layers. The Tukey’s test showed that there was no significant difference, at the 5% level, between the time value used and the mean value modeled for training, testing, and validation for the NaCl. For the KCl, a significant difference was observed only for 2 training samples and 1 test sample. Sensitivity analysis showed that the discrete variable Z, which represents the static and dynamic systems, was the most important in the models’ construction.

Abstract A highly efficient tool that has stood out in data processing and treatment is Artificial Neural Networks (ANNs). Self-organized maps (SOM), which is a type of ANNs, are easy-to-view computational models that simulate information processing and knowledge acquisition. Applying this tool, we analyzed the behavior of the film formed on the mozzarella cheese surface when subjected to salting by immersion. In order to reduce the amount of NaCl in the food, KCl was used as a partial substitute in adequate amounts, without causing sensory changes in the cheese. The diffusion of salts in the brine is caused by the transfer of sodium and potassium ions, induced by concentration gradient of these salts inside and outside the food. The application of artificial neural networks proved to be an effective tool in the evaluation of the influence of the film on the diffusion process of Na+ and K+ ions, in the mozzarella cheese salting.

RESUMO O presente trabalho problematiza a hipervalorização da branquidade e suas implicações na vida em sociedade, principalmente no que toca à condição de subalternização do povo negro. O corpus de discussão será um veículo da mídia televisiva, o quadro RJ-Móvel, transmitido pelo diário telejornalístico local RJTV, da Rede Globo de Televisão. O quadro aborda situações em que uma jornalista, juntamente com sua equipe, tenta mediar e solucionar problemas relativos à falta de infraestrutura em bairros periféricos da cidade do Rio de Janeiro, bem como na região do chamado Leste Metropolitano e na Baixada Fluminense. Analisaremos a construção do roteiro do quadro e sua inserção em meio à população negra e pobre, maioria das(os) residentes nestes locais e sobre as(os) quais o programa se debruça, destinando suas investidas de caráter assistencialista. Como complemento de análise deste objeto, onde se observam outros modos de operar práticas de racismo e estabelecer lógicas de condutas civilizatórias, faremos uma leitura crítica também a respeito da postura da apresentadora, seus movimentos e dinâmicas de interação com a população local. Para tanto, traçaremos um diálogo com a noção de amabilidade artificiosa, mobilizada por Fanon (2008) em Pele Negra, Máscaras Brancas, em aproximação com duas outras categorias: a primitivização da pessoa negra e a caricata espetacularização da negritude e da pobreza. Assim nos interessa refletir estratégias que repensem o enaltecimento da branquidade, rompendo com o discurso midiático hegemônico que performa assistencialismo, produz e reproduz - entre outros modos de estereotipação - práticas racistas.

ABSTRACT The present work discusses the overvaluation of whiteness and its implications for life in society, especially with regard to the condition of subordination of black people. The corpus of discussion will be a television media vehicle, the RJ-Móvel, broadcast by the daily local television newscast RJTV, of Rede Globo. RJ-Móvel addresses situations in which a journalist, together with her team, tries to mediate and solve problems related to the lack of infrastructure in peripheral neighborhoods of Rio de Janeiro, as well as in the so-called Leste Metropolitano and in the Baixada Fluminense. We will analyse the construction of the script and its insertion among the black and poor population, most of them residents in these places and on whom RJ-Móvel focuses, assuming an assistance character. As a complement to the analysis of this object, where other ways of operating racism practices and establishing logics of civilizing behavior are observed, we will also make a critical reading about the reporter’s posture, her movements and interaction dynamics with the local population. For that, we will establish a dialogue with the notion of artificial kindness, mobilized by Fanon (2008) in Black skin, white masks, in approximation with two other categories: the primitivization of the black person and the spectacularization of blackness and poverty. Thus, we are interested in reflecting on strategies that make us rethink the praise of whiteness, breaking with the hegemonic media discourse the performs assistance, produces and reproduces racist practices, among other modes of stereotyping.

RESUMO Este artigo objetiva analisar o modo como o Colégio Pedro II, no Rio de Janeiro, implementa a política de cotas nos processos de admissão de alunos para os diferentes segmentos da educação básica. Em suma, indagamos se a política de cotas se traduziu na presença de um maior quantitativo de pessoas negras no corpo discente da instituição. A partir do estudo dos editais de seleção discente e da análise do perfil racial dos alunos, constatamos que, a despeito de o Colégio Pedro II aplicar a política de cotas raciais em seus processos seletivos, a instituição o faz apenas para o Ensino Médio (conforme prevê, em sentido estrito, a Lei n. 12.711/2012), não contemplando os demais segmentos da educação básica e, portanto, pouco modificando o perfil racial do corpo discente nos últimos anos.

ABSTRACT This article aims to analyze how Colégio Pedro II, in Rio de Janeiro, Brazil, implements the policy of quotas in student admission processes for different segments of basic education. In short, we investigate whether the policy of quotas resulted in the presence of a greater number of black people in the institution’s student body. Based on the study of the civil service examination announcements for the selection of students and on the analysis of the student’s racial profile, we found out that, even though Colégio Pedro II applies the policy of racial quotas in its announcements, the institution does so only for secondary education (as provided by Law n. 12,711/2012, in the strict sense). That is to say the institution is not covering the other segments of basic education and, therefore, there is little change in the racial profile of the student body in recent years.

RESUMEN Este artículo tiene como objetivo analizar cómo el Colegio Pedro II, en Río de Janeiro, Brasil, implementa la política de cuotas en los procesos de admisión de estudiantes para diferentes segmentos de la educación básica. En resumen, preguntamos si la política de cuotas ha resultado en la presencia de una mayor cantidad de negros en el alumnado de la institución. Con base en el estudio de los avisos de selección de estudiantes y en el análisis del perfil racial de los estudiantes, encontramos que, aunque el Colegio Pedro II aplique de la política de cuotas raciales en sus procesos de selección, lo hace solamente en la educación secundaria (de acuerdo con la Ley n. 12.711, 2012, en sentido estricto), descuidando de los demás segmentos de la educación básica y, por lo tanto, modificando poco el perfil racial del alumnado en los últimos años.

OBJECTIVE: In 2020, the COVID-19 pandemic brought a work and stress overload to healthcare workers, increasing their vulnerability to mental health impairments. In response, the authors created the COMVC-19 program. The program offered preventive actions and mental health treatment for the 22,000 workers of The Hospital das Clinicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). This paper aims to describe its implementation and share what we have learned from this experience. METHODS: Workers were able to easily access the program through a 24/7 hotline. Additionally, a mobile phone app that screened for signs and symptoms of emotional distress and offered psychoeducation and/or referral to treatment was made available. Data from both these sources as well as any subsequent psychiatric evaluations were collected. RESULTS: The first 20 weeks of our project revealed that most participants were female, and part of the nursing staff working directly with COVID-19 patients. The most frequently reported symptoms were: anxiety, depression and sleep disturbances. The most common diagnoses were Adjustment, Anxiety, and Mood disorders. CONCLUSIONS: Implementing a mental health program in a multimodal intervention was feasible in a major quaternary public hospital. Our data also suggests that preventive actions should primarily be aimed at anxiety and depression symptoms, with a particular focus on the nursing staff.

RESUMO A urticária multiforme é uma reação de hipersensibilidade aguda, sendo um subtipo morfológico de urticária. É mais comum em crianças e, uma vez que sua apresentação clínica se assemelha a outras dermatoses, como o eritema multiforme, a vasculite urticariana e a doença do soro-like, tem sido subdiagnosticada. Relatamos assim, um caso de uma criança de 1 ano e 9 meses com lesões cutâneas exuberantes associado a manifestações sistêmicas, com recuperação completa do quadro após tratamento com anti-histamínicos e corticosteróides endovenoso em unidade de medicina intensiva.

ABSTRACT Urticaria multiforme is an acute hypersensitivity reaction which can be considered a morphological subtype of urticaria. It is more common in children and as its clinical presentation resembles other pathologies such as erythema multiforme, urticarial vasculitis and serum-sickness-like reaction, it has been often underdiagnosed. We report a case of a 1-year and 9-month-old child with exuberant skin lesions associated with systemic manifestations with complete and quick recovery after H1-antihistamines and iv steroids in an intensive care unit.

Abstract Problems related to high blood pressure have led consumers to choose foods with low levels of sodium chloride, and an alternative to reduce the salt content of this salt is its partial replacement with KCl. In order to not affect the sensory properties of cheese, salting was performed using a solution containing 70% of NaCl and 30% of KCl in a static and dynamic system. During the salting process by immersion, the mass transfer is affected by the formation of a film on the cheese surface. The analysis of the diffusion of salts in the film can be performed using self-organizing map (SOM) combined with the Finite Element Method (FEM). Through these tools, a greater influence of the film was observed when the static system was applied and that the diffusion is different according to the studied position. On the biosolid sides the diffusion was more pronounced than in the center, indicating a decrease in the film thickness towards the edges. The SOM combined with 3D modeling showed to be an efficient tool to investigate the formation, influence and behavior of the film during the diffusion of Na+ and K+ on salting of Prato cheese.

SUMMARY: The aim of this study was to describe the genotypic and allele frequency of ACTN3 R577X and ACE I/D in blind athletes of 5-a-side football performance. A descriptive methodology was included with a sample of 63 blind male athletes (28.0 ±5.8 years) of football teams with a 5-a-side performance rating. The polymorphism was determined by means by of real-time Polymerase Chain Reaction (rt-PCR). Statistics were descriptive based on the measures of frequency of genotypes and alleles. The genotypic frequency of ACTN3 by the athletes presented the following distribution: 28.6 % with RR genotype, 54 % with RX and 17.4 % XX and allele frequency of 55.6 % for the R allele and 44.4 % for the X allele. As for ACE I/D, the genotype frequency was 63.5 % for genotype ID, 22.2 % for DD and 14.3 % for II. The allele frequency showed a predominance of the D allele with 53.9 %. The study found for ACTN3 R577X and ACE I/ D of blind athletes of 5-a-side football, a predominance of genotypes and alleles representative of strength and speed modalities.

RESUMEN: El objetivo de este estudio fue describir la frecuencia genotípica y alélica del ACTN3 R577X y ECA I/D en atletas ciegos de fútbol 5. Se incluyó una metodología descriptiva con una muestra de 63 deportistas ciegos (28,0±5,8 años), todos varones, de equipos de fútbol 5 de alto rendimiento. El polimorfismo se determinó mediante la reacción en cadena de la polimerasa en tiempo real (RT-PCR). La estadística fue descriptiva realizada a partir de las medidas de frecuencia de genotipos y alelos. La frecuencia genotípica de la ACTN3 en los deportistas presentó la siguiente distribución: el 28,6 % con genotipo RR, el 54 % con RX y el 17,4 % con XX y frecuencia alélica del 55,6 % para el alelo R y del 44,4 % para el alelo X. En cuanto a la ECA I/D, la frecuencia genotípica fue del 63,5 % para el genotipo ID, del 22,2 % para el DD y del 14,3 % para el II. La frecuencia alélica presentó prevalencia del alelo D con el 53,9 %. El estudio constató una predominancia de los genotipos y alelos representativos de las modalidades de fuerza y velocidad para ACTN3 R577X y ECA I/D de atletas de fútbol 5.

RESUMO A saúde ambiental se constitui o campo da saúde pública responsável pelas políticas públicas relacionadas com a interação entre a saúde humana e os fatores do meio ambiente. A gestão de risco de desastres envolve um processo de antecipação, planejamento e preparação para resposta, com atuação de diferentes áreas de governo, como o setor saúde. Este relato de experiência teve o objetivo de descrever as ações desenvolvidas pela vigilância em saúde ambiental da Secretaria de Estado de Saúde de Minas Gerais em resposta ao rompimento da barragem B1 em Brumadinho. Entre as ações desenvolvidas, destacam-se o monitoramento da qualidade da água para consumo humano nos municípios atingidos, a participação em reuniões em espaços de decisão e deliberação e a produção de documentos técnicos. As experiências vivenciadas pela equipe, durante as ações de resposta ao desastre, motivaram reflexões sobre a necessidade de fortalecer o desenvolvimento das ações de forma intra e intersetorial e a importância de qualificação dos dados registrados nos sistemas de informação e de conhecimento das informações locais de cada território, a fim de subsidiar a atuação do setor saúde de modo oportuno em situações de desastre.

ABSTRACT Environmental health is the field of public health responsible for public policies related to the interaction between human health and environmental factors. Disaster risk management involves a process of anticipating, planning, and preparing for a response, with actions from different areas of the government, such as the health sector. This experience report had the objective of describing the actions developed by the environmental health surveillance of the Minas Gerais State Department of Health in response to the rupture of the B1 dam in Brumadinho. Among the actions developed, the monitoring of the quality of water for human consumption in the affected municipalities, the participation in meetings in spaces for decision and deliberation, and the production of technical documents are highlighted. The experiences lived by the team, during the disaster response actions, motivated reflections on the need to strengthen the development of the actions in an intra and intersectoral way, as well as the importance of qualifying the data recorded in the information systems, and knowledge of the local information of each territory, in order to subsidize the performance of the health sector in a timely manner in disaster situations.
